75岁以上老年患者微创经椎间孔腰椎体间融合术疗效评价Minimally invasive transforaminal lumbar interbody infusion for one- level degenerative disease in patients over 75

摘要(Abstract):
[目的]探讨微创经椎间孔腰椎体间融合术(minimally invasive transforaminal lumbar interbody fusion,MISTLIF)治疗75岁以上老年人单节段腰椎退性行疾病的临床效果。[方法]回顾性分析2009年6月~2012年6月,因单节段腰椎管狭窄症、腰椎间盘突出症接受腰椎融合手术治疗的75岁以上患者41例,18例接受直视下MIS-TLIF,23例接受传统开放经椎间孔腰椎体间融合术(Open-TILF),术后随访2年,比较两组临床效果及并发症。[结果]两组一般资料比较差异无统计学意义(P>0.05);MIS组术后发生并发症1例(5.6%),Open组7例(30.4%),两组间差异有统计学意义(P<0.05);MIS组术中出血量、术后引流量、下地时间和术后住院时间均低于Open组(P<0.01),术后5 d MIS组腰痛VAS评分优于Open组(P<0.05),随访期间两组间ODI评分、腰腿痛VAS评分均明显优于术前(P<0.05),术后2年融合率差异无统计学意义(P>0.05)。[结论]75岁以上老年人MIS-TLIF手术临床效果肯定,创伤小、恢复快、术后并发症少。
