摘要(Abstract)：

[目的]探讨80岁及以上膝骨关节炎患者行初次全膝关节置换术后1个月内急性合并症和并发症的发生情况,并分析相关危险因素,为临床实践提供参考。[方法]回顾性分析2012年1月～2018年7月在本院因膝骨关节炎行初次单侧全膝关节置换术的90例高龄患者的资料。男性36例,女性54例,年龄80～92岁,平均(81.84±2.54)岁。收集并记录患者一般资料、既往手术史、术前合并症、手术时间、术中出血量、麻醉方式、实验室检查数据及术后1个月内急性合并症和并发症发生情况,分析相关危险因素。[结果]该组患者术后1个月内共发生手术相关并发症4例(4.44%),其中急性心肌梗死伴急性左心衰1例,手术切口感染伴下肢深静脉血栓形成1例,术后谵妄2例,所有患者均经积极治疗后好转,未出现死亡病例。统计分析显示术前高血糖(P=0.039)和术前合并脑部疾病(P=0.047)是术后1个月内发生并发症的危险因素。[结论]高龄骨关节炎患者全膝关节置换术后有发生下肢深静脉血栓形成、切口感染、心肌梗死及谵妄等并发症的可能。术前合并血糖高或脑部疾病是术后1个月内发生并发症的独立危险因素。
