双Endobutton钢板与肩锁钩板治疗急性完全性肩锁关节脱位的临床对比研究(英文)Double endobutton compared with hook plate for treatment of acute complete acromioclavicular joint dislocations

摘要(Abstract):
[目的]研究双Endobutton技术在治疗肩锁关节脱位中维持关节复位和改善肩关节功能的疗效。[方法]本前瞻性研究依托江苏大学附属医院,2011年7月~2013年8月在本院就诊的急性Tossy III型肩锁关节脱位患者共30例,随机分成两组,分别采用双Endobutton技术(DE组)或锁骨钩板(HP组)固定,每组各15例,并跟踪随访1年。记录两组患者手术时间、并发症、住院时间等围手术期指标,并分别于术前及术后1、3、6、12个月记录患者的X线片、视觉模拟评分(VAS)和Constant-Murley评分进行统计学分析。[结果]术后X线片显示两组患者均获得了满意的肩锁关节复位。术前及术后1、3、6、12个月两组的VAS评分(P=0.886 9,0.580 3,0.8228,0.100 3,0.706 2)及Constant-Murley评分(P=0.458 1,0.379 4,0.151 2,0.165 0,0.813 4)均没有明显差异。HP组的手术时间与DE组相比明显较短(P<0.000 1),但是其并发症的发生率却较高(P=0.017 6)。[结论]双Endobutton技术在治疗急性肩锁关节全脱位上具有与锁骨钩板相似的疗效,而这项新技术的优点是允许患者术后早期锻炼并无需二次手术取出内固定物。
