神经阻滞下手法松解矫正膝关节置换术后屈曲挛缩Continuous proximal sciatic nerve blockade in the treatment of flexion contractures after primary TKA

摘要(Abstract):
[目的]本研究旨在利用便携式步态分析仪了解全膝关节置换术后应用持续近端坐骨神经阻滞结合康复训练治疗屈曲挛缩患者膝关节置换前后的步态参数变化及特点,指导术后的康复治疗。[方法]通过便携式步态分析仪对2011年3月~2014年8月间收治的20例术后经过一个半月的传统康复训练仍存在15°以上屈曲挛缩患者作为研究组,同时选取同时期术后未发生屈曲挛缩的20例患者作为对照组,在阻滞前及阻滞后半年进行步态分析测试。[结果]屈曲挛缩组坐骨神经阻滞治疗后6个月步长(Step Length)、步速(Speed)、步幅持续时间(Step Duration)、单腿支撑时间与双腿支撑时间之比(SLS/DLS)、拔腿强度(Pulling Accel)、摆腿强度(Swing Power)、地面反冲(Ground Impact)、足落地控制(Footfall)及行走中活动度(Angle)均有明显改善。[结论]①持续近端坐骨神经阻滞结合康复训练对于术后出现难以通过传统康复训练治疗的持续屈曲挛缩有明显疗效,通过治疗患者的行走功能明显改善。这种方法可以作为治疗膝关节置换术后屈曲挛缩的一种有益的补充;②PMA可以客观地记录全膝关节置换患者术前及术后的步态参数,反映膝关节的功能状态,并对评价手术效果及指导功能康复提供了相对客观和一致性的标准。
