功能锻炼与常规干预对腰椎术后临床结果影响的荟萃分析Effect of functional exercise versus conventional intervention on clinical outcomes of lumbar surgeries: a meta-analysis

摘要(Abstract):
[目的]系统评价临床常见腰椎手术术后进行系统功能锻炼结合传统术后干预与单纯常规干预对康复效果的影响。[方法]检索Pubmed、Cochrane Library、Em-Base、Web of Science、Ovid、中国期刊全文数据库(CNKI)、万方数据知识服务平台、维普中文期刊服务平台,检索时间均为1994年1月~2018年7月。本研究提取Oswestry功能障碍指数(ODI),视觉模拟评分法(visual analogue scale, VAS),贝克抑郁自评量表(BDI);健康调查简表(SF-36),日本骨科协会评估治疗分数(JOA)为评价指标。采用RevMan5.3以及SPSS 21软件进行数据分析。[结果]经评估筛选, 15篇文章纳入本研究,共1049例患者,其中系统功能锻炼组520例,常规术后康复组529例。锻炼组ODI评分[MD=-9.46,95%CI1.08),P<0.05]低于常规组。两组BDI抑郁评分差异无统计学意义[MD=-1.74,95%CI(-4.32~-0.83),P>0.05]。(0.61~1.26),P<0.05]与JOA评分[MD=5.26,95%CI(4.50~6.01),P<0.05]显著高于常规组。[结论]腰椎手术术后进行系统的功能训练对患者的生理状态恢复具有积极作用。
