根据损伤应力顺序复位固定治疗旋前外旋型踝关节骨折

摘要(Abstract)：
[目的]探讨根据损伤应力顺序切开复位内固定治疗旋前外旋型踝关节骨折的临床疗效。[方法]对2014年3月～2017年12月收治的37例旋前外旋型踝关节骨折进行回顾性研究,男27例,女10例;年龄18～64岁,平均(39.14±9.47)岁。按照Lange-Hansen分型:旋前外旋型Ⅲ度损伤15例, IV度损伤22例。所有骨折均根据损伤应力顺序由内向外进行复位固定。采用美国足踝外科协会(AOFAS)踝-后足评分和视觉模拟评分(VAS)对临床结果进行评估。[结果]所有患者切口均一期愈合,无感染、皮肤坏死等并发症,37例患者全部随访,随访时间12～24个月,骨折愈合时间为10～17周。AOFAS评分由术前的(45.23±5.63)分提高至末次随访时的(87.88±6.21)分(P<0.001),VAS评分由术前的(7.68±3.12)分降至末次随访时的(2.11±1.45)分(P<0.001),无踝关节不稳定表现。[结论]对旋前外旋型踝关节骨折根据损伤应力顺序行切开复位内固定治疗,骨折复位容易,固定坚强,术后关节功能恢复良好,临床效果满意。
