摘要(Abstract)：

[目的]探讨关节镜下骨折块复位、使用多枚带线锚钉缝合固定前交叉韧带(ACL)胫骨止点撕脱骨折的手术方法及临床疗效。[方法]2010年1月~2013年6月,对24例ACL胫骨止点撕脱骨折患者在关节镜下进行骨折复位,2至3枚带线锚钉经ACL基底部缝合固定骨折块。术前胫骨髁间嵴撕脱骨折的Meyers-Mc Keever分型:Ⅱ型6例,Ⅲ型8例,Ⅳ型10例;男16例,女8例;年龄21~57岁,平均28.5岁。比较患者手术前后的前抽屉试验、Lachman试验、Lysholm评分和IKDC 2000主观膝关节评分。[结果]随访时间8~48个月,平均28.5个月,1例失访。术后X线片骨折均复位良好,术后3个月X线片示胫骨髁间嵴骨折均愈合,无锚钉拔出及再骨折病例。末次随访时,患膝活动度0°~140°,均无疼痛;前抽屉试验及Lachman试验均由术前阳性转为阴性;Lysholm评分和IKDC2000主观膝关节评分分别由术前的(47.5±3.7)分、(52.4±5.3)分,改善为随访时的(88.6±3.2)分、(91.8±5.7)分,两项评分手术前后比较差异均有统计学意义(t1=22.100,t2=20.700,P<0.01)。[结论]关节镜下使用多枚带线锚钉缝合固定胫骨髁间嵴撕脱骨折具有手术创伤小、骨块固定牢固、利于早期功能锻炼及无需二次手术取出内固定等优点,对于Mevers-Mc KeeverⅣ型骨折及骨折块小而薄或者骨骺未闭合者尤其适用。
