骨质疏松脊柱压缩性骨折经皮穿刺椎体成形术后再骨折因素的临床研究Clinical study of refractures factors of osteoporotic vertebral compression fractures treated by percutaneous vertebroplasty

摘要(Abstract):
[目的]探讨经皮穿刺椎体成形术(percutaneous vertebroplasty,PVP)治疗骨质疏松性脊柱压缩性骨折(osteoporosis vertebral compression fracture,OVCF)后再骨折的相关危险因素。[方法]回顾性分析经皮穿刺椎体成形术治疗骨质疏松压缩性骨折患者264例(394个椎体),分为A组(再发骨折组)21例,B组(未再发骨折组)243例。随访时间平均(16.50±6.50)个月,记录术后再发骨折的时间、部位及发生率,记录患者的年龄、性别、穿刺方法、骨水泥量、原发椎体平均数目、QCT值、骨水泥渗漏、椎体高度恢复率、手术前后VAS评分、椎体后凸角等因素并对以上因素进行分析。[结果]所有患者均成功完成手术,椎体成形术后首次再发骨折的发生率为7%,时间为(9.34±8.34)个月,第2、3次发生在3个月内。骨水泥渗漏率为9%,渗漏后无临床症状发生。年龄、性别、穿刺方法、骨水泥量、原发椎体平均数目、骨水泥渗漏、椎体高度恢复率、椎体后凸角度(Cobb)等因素在A组和B组间差异无统计学意义(P>0.05);术前及术后第3 d VAS评分在A组、B组组内比较有显著性意义(P<0.05),终末随访VAS评分在两组间对比有统计学意义,P<0.05;QCT值在A组和B组间有显著性意义(P<0.05)。[结论]骨质疏松性脊柱压缩性骨折患者行椎体成形术治疗,能够取得满意效果,再发骨折发好于术后9个月内,骨密度是PVP后再发骨折的一个危险因素。
