稳定机制在经皮球囊扩张椎体成形术治疗椎体压缩性骨折疗效中的作用Effect of stability mechanism on osteoporotic vertebral compression fractures treating with percutaneous kyphoplasty

摘要(Abstract):
[目的]探讨稳定机制在经皮球囊扩张椎体成形术(percutaneous kyphoplasty,PKP)治疗骨质疏松性椎体压缩性骨折疗效中的作用。[方法]2013年6月~2015年2月,按照纳入与排除标准,纳入本研究的椎体压缩性骨折病例共50例(50个椎体),行PKP术治疗,应用"弹簧压缩法"评估患者入院后、术中球囊扩张0、0.5、1.0、1.5 ml、最终状态及骨水泥硬化后这7个阶段的VAS评分,通过比较VAS评分来研究球囊扩张体积与患椎稳定状态的关系。[结果]所有患者均取得7个阶段的VAS评分。椎体骨折复位达到满意状态时的球囊扩张体积平均为3.1 ml。在球囊扩张0.5、1.0 ml状态时与在扩张0 ml时的VAS评分差异无统计学意义(P>0.01;t_(0.5 ml)=1.208,t_(1.0 ml)=2.487);在球囊扩张1.5 ml、最终状态时与在扩张0 ml时的VAS评分差异有统计学意义(P<0.01;t_(1.5 ml)=19.228,t_(最终状态)=30.746);在骨水泥硬化后与球囊扩张达最终状态时的VAS评分差异无统计学意义(P>0.01,t_(骨水泥)=1.892)。[结论]PKP手术治疗椎体压缩性骨折能有效缓解疼痛,稳定机制对其疗效起主要作用。
