摘要(Abstract)：

[目的]探讨270°椎管减压椎体前中柱重建治疗严重胸腰椎骨折的疗效。[方法]回顾分析2010年1月～2016年12月收治的46例严重胸腰椎骨折患者。根据减压范围,分为270°椎管减压组19例,360°椎管减压组27例。比较两组围手术期、随访与影像资料。[结果]所有患者均顺利完成手术,无严重并发症。270°组手术时间、术中出血量、术后引流量及异体血输入量均显著少于360°组(P<0.05)。随访18～60个月,平均(45.34±10.41)个月。两组末次随访时VAS评分均较术前显著减少(P<0.05),但相同时间点两组间差异均无统计学意义(P>0.05)。两组患者末次随访AISA分级均较术前显著改善(P<0.05),相同时间点两组间差异均无统计学意义(P>0.05)。影像评估方面,与术前相比两组术后椎体前缘高度显著增加(P<0.05),Cobb角显著减小,椎管占位率显著减小(P<0.05)。相同时间点,两组间椎体前缘高度、伤椎Cobb角及椎管占位率的差异均无统计学意义(P>0.05)。至末次随访时,两组损伤节段均达到骨性融合,未出现内固定失败。[结论]对于严重胸腰椎骨折,行270°与360°椎管减压均可彻底减压,取得良好的临床效果。但270°椎管减压具有手术时间短,术中出血、术后引流量及异体血输入量少及脊柱稳定性破坏小等优点。
