关节融合与内固定治疗Lisfranc损伤的荟萃分析Primary arthrodesis versus open reduction and internal fixation for primary Lisfranc injury: A meta-analysis

摘要(Abstract):
[目的]系统评价初次lisfranc损伤关节融合与切开复位内固定治疗的疗效,为临床实践与研究提供参考。[方法]计算机检索Cochrane library、Pub Med、Embase、OVID、万方数据库、中国期刊网全文数据库、维普数据库、中国生物医学数据库,末次检索时间为2017年5月,选择适合本研究特点的质量评价标准对纳入文献进行质量评价,采用Rev Man 5.3软件对纳入文献进行合成分析。[结果]最终纳入2篇随机对照试验,4篇回顾性试验,总共纳入患者202例,切开复位内固定组患者107例,关节融合组患者95例。2组术后并发症(OR=0.86,95%CI=0.46~1.60,P=0.64)、术后AOFAS评分(MD=-17.15,95%CI=-41.02~6.73,P=0.16)、再次手术率(OR=4.07,95%CI=0.99~16.65,P=0.05),差异没有统计学意义;术后内固定取出率(OR=11.28,95%CI=4.20~30.33,P<0.001)、术后满意率(OR=0.24,95%CI=0.08~0.73,P=0.01),差异有统计学意义。[结论]关节融合组术后满意度高于内固定组,术后内固定取出率低于内固定组,内固定组与关节融合组术后并发症、术后再次手术、AOFAS评分相比较差异无显著性意义。
