青少年前交叉韧带止点撕脱骨折的保护性治疗

摘要(Abstract):
[目的]探讨青少年前交叉韧带(ACL)胫骨止点撕脱性骨折的保护性治疗策略与临床效果。[方法]2014年8月~2018年3月对18例青少年前交叉韧带止点撕脱性骨折患者分别应用2根双股Ultrabraid线活扣套扎ACL,借助前叉定位器在骨槽边缘的前内及前外钻入2.0 mm克氏针后形成胫骨隧道,平面位于胫骨平台骺板以上,分别经直径2.0 mm骨隧道将Ultrabraid线引出后沿骨表面下行至胫骨骺板平面下方、胫骨结节内侧PUSHLOCK螺钉加压内固定。[结果]所有患者均顺利完成手术,术中关节镜下所有患者撕脱骨折均原位复位。随访时间为6~24个月,平均(13.72±1.07)个月。所有病例末次随访均达到影像学上的骨性愈合, Lysholm评分平均为(95.43±1.62)分,前抽屉试验阴性,KT-1000前移差值平均为(1.22±0.81) mm,IKDC评分18例均为A类。[结论]对青少年ACL止点撕脱骨折采用2条Ultrabraid线活扣套扎,避开骺板的胫骨隧道技术联合PUSHLOCK锚钉加压内固定技术复位固定,安全可靠,不损伤骨骺,术后功能恢复良好。
