股骨转子间骨折PFNA内固定术后隐性失血及其相关因素分析Postoperative hidden blood loss and its contributing factors in femoral intertrochanteric fracture patients treated with proximal femoral nail anti-rotation

摘要(Abstract)：
[目的]验证股骨转子间骨折患者使用股骨近端防旋髓内钉(proximal femoral nail anti-rotation,PFNA)内固定术后隐性失血的存在,并探讨隐性失血与各因素之间的相关性。[方法]按照纳入和排除标准,选取2013年1月~2015年2月期间在广东省中医院骨科住院的患者共208例。记录患者围手术期的各种资料,利用Gross方程和Nadler方程,计算出术后隐性失血量,并探讨其与性别、年龄、体重指数(BMI)、骨折类型、手术用时、糖尿病、高血压病、术前时间等因素的相关性。[结果]平均手术用时(69.76±21.67)min,出现HCT最低的时间是术后(3.21±1.04)d,实际失血总量(773.16±289.65)ml,显性失血总量(114.77±62.41)ml,隐性失血量(658.39±259.15)ml。隐性失血占了总失血量的84.7%,且与骨折类型、性别、糖尿病、手术用时、高血压病等因素显著相关(P<0.05),与患侧、术前时间、年龄和体重指数无关。[结论]股骨转子间骨折患者行PFNA内固定术后存在较多的隐性失血,且与骨折类型、性别、糖尿病、手术用时、原发性高血压等因素有显著的相关性。
