膝关节置换术后加压包扎减轻肿胀的有效性和安全性荟萃分析Efficacy and safety of pressure dressing on knee swelling after total knee arthroplasty: a meta-analysis

摘要(Abstract):
[目的]系统评价膝关节置换术后下肢加压包扎减轻膝关节肿胀的有效性和安全性。[方法]计算机检索PubMed、Embase、The Cochrane Library等数据库,搜集膝关节置换术后下肢加压包扎的随机对照试验,检索时限均为建库至2018年10月。采用Stata14.0软件进行荟萃分析,采用GRADE pro3.6软件进行证据质量评价。[结果]共纳入6个RCT,荟萃分析结果显示:与TKA术后下肢常规包扎组相比,下肢加压包扎不能降低膝周径[MD=0.10,95%CI(-0.79,0.99),P=0.832]、大腿周径[MD=0.18,95%CI(-1.01,1.38),P=0.763]、小腿周径[MD=0.05,95%CI(-0.77,0.87),P=0.904]、术后第1 d VAS[MD=0.21,95%CI(-0.28,0.70),P=0.396]和术后第2 d VAS[MD=0.04,95%CI(-0.22,0.30),P=0.496]。证据质量评价显示:3个关键结局指标为中等质量,2个重要结局指标分别为中等质量和低质量。[结论]当前证据显示TKA术后下肢加压包扎并不能有效减轻膝关节肿胀和疼痛,反而会增加病人不适感。
