外固定支架联合锁定钢板内固定治疗C型桡骨远端骨折External fixation combined with locking plate fixation for type C distal radius fracture

摘要(Abstract):
[目的]探讨外固定支架联合锁定钢板内固定治疗C型桡骨远端骨折的临床效果。[方法]以本院2018年收治的60例患者为研究对象,并根据随机方法划分为联合固定组(30例)与外固定组(30例),其中联合固定组施以外固定支架联合锁定钢板内固定治疗,而外固定组则施以外固定支架治疗。比较两组围手术期资料、临床和影像结果。[结果]两组患者均顺利手术,两组均无神经血管损伤等严重并发症。虽然联合固定组在手术时间、术中失血量和切口长度显著大于外固定组(P<0.05),但两组在透视次数的差异无统计学意义(P>0.05)。联合固定组的术后去除外固定架的时间显著早于外固定组(P<0.05)。术后12个月两组患者各向活动的差异无统计学意义(P<0.05),但联合固定组的VAS评分和Gartland-Werley评分显著低于外固定组(P<0.05)。影像评估方面,联合固定组的术后桡骨远端尺偏角、掌倾角和桡骨茎突高度均显著大于外固定组(P<0.05),联合固定组的骨折愈合时间显著早于外固定组(P<0.05)。[结论]外固定支架联合锁定钢板内固定治疗C型桡骨远端骨折的临床效果优于单纯外固定。
