植骨与骨水泥椎弓钉强化治疗强直性脊柱炎胸腰段骨折的比较Bone graft versus cement for pedicle screw augmentation in treatment of thoracolumbar fracture in patients with ankylosing spondylitis

摘要(Abstract):
[目的]探讨钉道内植骨强化、增大椎弓根螺钉外展角和骨水泥强化椎弓根螺钉在治疗强直性脊柱炎(AS)胸腰段骨折中的临床效果。[方法]2009年4月~2015年7月本院采用椎弓钉-棒内固定治疗AS脊柱胸腰段骨折患者52例。其中,30例采用增大椎弓根螺钉外展角和椎弓根钉道内植骨增加椎弓钉的把持力(植骨组),而其余22例采用骨水泥强化椎弓根螺钉(骨水泥组)。对术中情况(手术时间、失血量)、术后引流量、住院时间、卧床时间、术后并发症(伤口感染,脂肪液化,肺部、泌尿系感染,深静脉血栓,褥疮,内固定松动)、术后VAS及ODI和JOA评分进行对比分析。[结果]两组在手术时间、失血量、引流量、住院时间、卧床时间差异无统计学意义(P>0.05);两组的伤口感染、脂肪液化、肺部感染、泌尿系感染、深静脉血栓、褥疮、内固定松动例数的差异无统计学意义(P>0.05)。随访10~18个月,平均14.2个月,术后两组VAS、ODI评分较术前明显降低,JOA评分较术前明显增高,差异均有统计学意义(P<0.05);但两组间相同时间点比较,差异无统计学意义(P>0.05)。[结论]椎弓根钉道内植骨、增大椎弓根螺钉外展角的增强方法和骨水泥增强椎弓根螺钉方法均可有效治疗强直性脊柱炎胸腰段骨折,两种方法的临床效果无差异。
