26例胸腰段压缩性骨折后路短节段内固定术后矫正丢失的观察与分析

摘要(Abstract)：
[目的]分析胸腰段压缩性骨折行后路短节段内固定术后区域矢状面参数在不同随访时间点的变化,探讨引起矫正丢失的原因。[方法]回顾性分析2010年1月～2011年10月江苏大学附属医院骨科收治的胸腰段单个椎体压缩性骨折患者26例,分别于术前、术后3 d内、术后3个月、末次随访时行胸腰段标准侧位X线片检查,测量其矢状面各参数:伤椎近端椎间盘高度(proximal intervertebral height,PIH)、伤椎远端椎间盘高度(distal intervertebral height,DIH)、伤椎前缘高度(anterior vertebral height,AVH)、椎体楔变角(anterior wedge angle,AWA)、节段性后凸角(regional angle,RA),运用SPSS 16.0进行统计学分析。[结果]所有患者均获得满意随访,平均18.6个月(12～24个月)。术后3个月时,PIH、AVH、AWA较术后3 d内分别丢失2.11 mm、3.52 mm、3.36°,差异有统计学意义(P<0.05)。至终末随访时,PIH与术后3个月时相比进一步丢失0.51 mm,差异有统计学意义(P<0.05)。[结论]体位复位+后路短节段椎弓根螺钉固定是纠正胸腰段压缩性骨折后凸畸形的有效方法,但术后存在矫正丢失,近端椎间盘高度的丢失尤为明显,术中过度撑开可能会加重椎间盘的损伤,因此建议术中先行体位复位,以减少对椎间盘的过度撑开。
