前外侧联合后内侧手术入路治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折疗效观察

摘要(Abstract)：
[目的]评价经膝前外侧和后内侧手术入路治疗复杂胫骨平台骨折的临床疗效。[方法]选择本院2007年3月~2010年3月收住院的32例复杂胫骨平台骨折作为研究对象,均采用前外侧联合后内侧手术入路治疗,其中SchatzkerⅤ型24例,Ⅵ型8例,术后随访12~28个月,观察术后有无感染、皮肤愈合状况及骨折愈合时间,根据膝关节Rasmussen评分标准评定术后关节功能,配对t检验比较术后1周内胫骨平台内翻角(TPA)、后倾角(PA)与术后1年有无差异。[结果]2例浅表感染,4例深部感染,骨折平均愈合时间(4.83±0.69)个月。按照膝关节Rasmussen评分标准评定疗效,优9例,良17例,可4例,差2例,优良率81.2%,术后1周内TPA、PA与术后1年比较,差异无统计学意义(P>0.05)。[结论]膝关节前外侧和后内侧联合入路治疗复杂胫骨平台骨折可充分暴露膝关节内结构及胫骨平台关节面,可使关节面得到有效准确地复位,且具有良好的手术操作空间。术后关节面复位丢失率低,骨折愈合状况及术后关节功能恢复良好。
