镁硅玉髋关节假体周围严重骨缺损的修复与重建Reparation and reconstruction of severe bone defects in periprosthetic of hip

摘要(Abstract):
[目的]介绍和评价镁硅玉人工髋关节置换后假体周围严重骨缺损的修复与重建的方法。[方法]总结分析12例12髋假体周围严重骨缺损的翻修病例,男8例,女4例;年龄42~72岁,平均58·6岁。翻修方法:髋臼侧均采用异体颗粒骨移植加髋臼增强环罩,股骨侧7例采用定做的长柄股骨假体和现代骨水泥技术,5例采用了紧密颗粒骨植入技术和标准股骨假体。[结果]以上病例经3~5a随访,无1例发生再移位或再发骨吸收,Harris髋关节功能评分由术前的19~52分提高到54~89分。[结论](1)镁硅玉人工髋关节的设计存在不合理性,不能被作为人工髋关节置换的假体;(2)正确使用植骨和选择恰当的假体,可明显提高翻修手术的成功率。
