股骨机械-解剖轴夹角与颈干角、偏心距的相关性(英文)Correlation of femoral mechanical- anatomical angle with neck- shaft angle and offset

摘要(Abstract):
[背景]在全膝关节置换术(total knee arthroplasty,TKA)中,外科医生常选择外翻5°的固定角度作为股骨远端外翻截骨角(valgus cut angle,VCA)。为了获得术后理想的下肢力线,这一角度理论上应该等于股骨机械-解剖轴夹角(femoral mechanical-anatomical angles,FMAA)。但是FMAA存在变异,因此固定的VCA可能无法适应不同的FMAA,反而不利于力线的重建。[目的]描述FMAA的变异及探索相关因素。[方法]本研究分析了86例膝关节的下肢全长影像,测量了FMAA、颈干角(neck-shaft angle,NSA)、偏心距(offset)等数据分布并进行了相关性分析。[结果]24.4%的膝关节(21例)FMAA>7°或者<5°,FMAA与颈干角存在负相关性(r=-0.48,P=0.01),且与偏心距存在正相关性(r=0.67,P<0.05),男性FMAA平均5.83°明显高于女性FMAA平均5.47(P=0.03)。[结论]FMAA与NSA及偏心距之间存在显著相关性;在TKA术前对全长X线片进行测量评估,取得FMAA、NSA和偏心距作为参考,可使得VCA的选择变得更加合理、个体化。
