膝关节外侧盘状软骨全切后晚期髌骨外移Lateral displacement of the patella at later stage after arthroscopic total meniscectomy of lateral discoid meniscus

摘要(Abstract):
[目的]评价关节镜下盘状软骨全切术后晚期对髌骨轨迹的影响。[方法]选择自2004年1月~2009年12月收治的96例外侧盘状软骨损伤患者为研究对象。其中男44例,女52例;年龄14~38岁,平均(25.03±9.45)岁。左膝50例,右膝46例。所有患者均于关节镜下行盘状软骨全切手术。行负重位X线片和膝关节CT扫描检查,测量胫骨股骨角、外侧胫股关节间隙、髌骨外移度和胫骨结节滑车沟间距。手术前后数据对比观察,进行统计学分析。[结果] 96例患者术后均获得随访。平均随访时间(12.56±2.31)年。患者膝外侧关节间隙由术前(5.95±1.50) mm显著减少至末次随访时(3.95±0.85) mm,差异有统计学意义(P<0.05);CT检查髌骨外移度由术前(0.15±0.10) mm显著增加至末次随访时(3.25±0.60),差异有统计学意义(P<0.05);胫骨股骨角由术前(176.56±2.54)°显著减少至末次随访时(167.52±2.72)°,差异有统计学意义(P<0.05);TT-TG值由术前(11.60±2.85) mm显著增加至(20.35±2.25) mm,差异有统计学意义(P<0.05)。[结论]关节镜下外侧盘状软骨全切手术可能导致髌骨外移,引发髌股关节炎。
