胫骨平台骨折术后切口愈合不良的影响因素Risks factors of poor incision healing after open reduction and internal fixation for tibial plateau fractures

摘要(Abstract)：
[目的]分析影响手术治疗胫骨平台骨折切口愈合的相关因素。[方法]回顾性分析本院于2014年7月～2017年5月接受手术治疗的胫骨平台骨折患者170例。记录各研究对象性别、年龄、吸烟情况、术前是否行骨牵引、外伤至手术时间、骨折Schatzker分型、内固定材质、手术时间、手术切口选择及术后2周是否能顺利拆除切口缝线并愈合。采用多因素非条件Logistic回归分析影响切口愈合相关因素。[结果]170中切口愈合良好153例(愈合良好组),切口愈合不良17例(愈合不良组),两组单因素比较显示:吸烟、术前不行跟骨骨牵引、外伤至手术时间、手术时间长、骨折Schatzker分型复杂及切口选用,与切口愈合不良相关(P<0.05)。此外,多因素逻辑回归表明,吸烟(OR=4.884,P<0.05)、外伤至手术时间(OR=10.274,P<0.05)、骨折Schatzker分型复杂程度(OR=10.984,P<0.05)及切口选择(OR=0.068,P<0.05)是切口愈合不良的危险因素。[结论]吸烟、外伤至手术时间短、骨折Schatzker分型复杂和切口选择是切口愈合不良的危险因素。
