胫骨骨干端延长后对膝关节内压力的影响Effect of diaphyseal lengthening of tibia on knee joint reactive force

摘要(Abstract):
[目的]活体内测量胫骨骨干端延长后膝关节内压力的变化。[方法]16只周龄为8周的健康新西兰幼兔的左腿放置双侧外固定架,于胫骨中段截骨后以每天分2次共0.8mm的速度延长30%,16只动物平均分为2组,分别于延长结束时(8只兔子,周龄13周)及随后的5周(8只兔子,周龄18周)测量膝关节内压力。压力的测定是通过两侧超膝关节牵引器内装的压力感应器与跨膝关节固定的关节牵引距离测定器实现的。当牵引器逐渐牵引关节时,关节牵引的力量和距离就通过与测定器相连的显示器纪录下来。[结果]延长侧膝关节内压力在30%胫骨骨干端延长结束时,较对侧正常关节有显著的升高(44.4±7.8N对27.2±4.0N),并维持至随后的5周后(44.3±6.5N对31.3±3.0N)。[结论]30%胫骨骨干端延长能升高膝关节内压力,有造成关节软骨损伤的风险。
