经皮椎体成形术治疗骨质疏松性脊柱压缩骨折的意外处理及几点探讨Accident and some problems of percutaneous vertebroplasty in treatment of osteoporotic vertebral compression fractures

摘要(Abstract)：
[目的]探讨经皮椎体成形术(PVP)治疗骨质疏松脊柱压缩骨折术中突发意识不清及术后再发疼痛的解决方法。[方法]自2003年11月~2007年8月应用PVP治疗95例156个椎体,男31例,女64例,33例无外伤史。24 h后鼓励患者下地活动。[结果]所有患者术后即有疼痛明显缓解,局部麻醉术中突然出现意识不清3例,CT检查排除脑血管意外,约30 min~1 h后神志转为清醒。1例术后并发脑血管意外。5例患者7~30 d左右再发相邻部位椎体骨折引起的疼痛,经行再次手术后症状消失。其中1例行第3次手术。[结论]对于并发症多,心情紧张的患者宜采用静脉麻醉加局麻的方式,对于有肺部或腹部疾病者宜选择侧卧位。对有潜在骨折可能的椎体,可预防应用经皮椎体成形术。
