老年髋部骨折手术风险预测评分系统Preoperative risk prediction models for mortality and complications following hip fracture surgery in the elderly

摘要(Abstract):
髋部骨折对生命力衰弱的老年人是一巨大打击。目前认为老年髋部骨折属于亚急诊疾病,及早手术、减少卧床时间,是降低患者死亡率和并发症的重要措施。但老年人整体机能衰退,器官合并症多,麻醉和外科手术对患者的机体储备是第二次打击。如何判别患者能否耐受手术,手术风险如何,是及早手术还是进一步内科调整,是摆在临床医生面前、需要快速回答的问题。本文介绍几个针对老年髋部骨折患者的手术风险评估量表(如合并症指数、衰弱指数等问卷量表)。术前用量表对每个患者进行风险评分和百分比估算,有针对性的进行风险告知和预后判断,对提高知情同意和医疗安全,有重要的临床价值。
