人工全踝关节置换的早中期疗效分析Therapeutic effect analysis of Scandinavian total ankle replacement in early metaphase

摘要(Abstract)：
[目的]探索采用STAR非限制活动型假体进行全踝关节置换术治疗终末期踝关节炎的早中期临床效果。[方法]1999年3月~2004年5月,共实施STAR假体全踝关节置换术11例,男7例,女4例;年龄27~68岁,平均53.5岁;创伤性关节炎7例,骨性关节炎3例,类风湿性关节炎1例;病程9~60个月。按Kofoed踝关节评分系统对患者术前、术后疼痛程度、关节功能、活动度进行综合评分。[结果]随访资料完备者8例,随访时间14~76个月,平均38个月。8例患者术前Kofoed评分:6~48分,平均29分;术后Kofoed评分:58~95分,平均80分。术前踝关节疼痛评分:0~15分,平均7分;术后疼痛评分:35~50分,平均48分。1例患者术中内踝劈裂,行可吸收螺钉内固定后完成假体置换;1例患者切口延迟愈合。随访时摄X线片均未见假体松动或下沉。[结论]STAR非限制活动负重型假体全踝置换术可有效缓解踝关节疼痛,改善踝关节活动度,并矫正踝关节畸形,是治疗终末期踝关节病变的有效手段。
