重组人红细胞生成素在全髋关节置换中的应用Effect of rhEPO in Total Hip Replacement

摘要(Abstract)：
目的 :研究重组人红细胞生成素 (rhEPO)对全髋关节置换术后失血性贫血的作用及作用机制。方法 :2 0 0 1年 6～ 1 2月期间 ,选择 36例单侧全髋关节置换病人纳入本实验。其中男 2 0例 ,女 1 6例 ,年龄 6 0～ 75岁 ,平均 6 4.6岁。将病人随机分为 2组 ,实验组于手术前 4d开始皮下注射rhEPO ,每次 1 0 0 0 0IU ,隔日 1次 ,共 2周 ;用药期间同时补充铁剂 ;对照组不使用rhEPO ,仅补充铁剂。用药前后及手术前后 ,监测两组病人的网织红细胞、血红蛋白、红细胞比积。实验组同时监测血压、血小板、血钾及肝肾功能。结果 :实验组的网织红细胞、血红蛋白和红细胞比积测定值在用药后不同时点上均高于对照组 ,差异具有统计学意义 (P <0 .0 5)。实验组病人的血压、血小板、血钾、谷丙转氨酶、尿素氮测定值在治疗前后无统计学差异 (P >0 .0 5)。用药后 ,实验组有 1例皮疹 ,2例注射部位疼痛 ,均自行缓解。结论 :全髋关节置换病人在围手术期使用rhEPO ,能有效预防和治疗病人的贫血状态 ,药物的耐受性良好。rhEPO的作用机制在于促进原红细胞分裂增殖并维持其活力 ,治疗期间需同时补充铁剂。
