PFNA与解剖锁定钢板治疗股骨转子间骨折临床疗效分析The comparative study of PFNA and anatomic proximal femoral locking plate in the treatment of intertrochanteric fractures

摘要(Abstract):
[目的]分析应用股骨近端抗旋髓内钉(proximal femoral nail anti-rotation,PFNA)与解剖锁定钢板(an-atomic proximal femoral locking plate,ALP)治疗股骨转子间骨折的疗效。[方法]对本院2006年5月~2010年5月应用股骨近端抗旋髓内钉(PFNA组)、解剖锁定钢板(ALP组)2种手术方式治疗股骨转子间骨折(EvansⅠ~Ⅳ型)共103例进行回顾性总结分析。其中,PFNA组53例,ALP组50例。[结果]本组103例患者中,92例获得随访,随访12~48个月,平均37.2个月。(1)在手术时间、术中出血量、术后引流量方面,PFNA组手术时间短、术中出血量少、术后引流量少,两组比较差异有统计学意义(P<0.05);(2)在骨性愈合时间、术后并发症及术后髋关节功能(Harris评分)方面,两组差异无统计学意义(P>0.05)。[结论]PFNA与解剖锁定钢板均为治疗股骨转子间骨折的有效治疗方式;PFNA具有手术时间短、术中出血量少、术后引流量少之优势,对于身体状况较差、不能耐受较长时间手术者,可优先考虑此手术方式。
