肱骨近端骨折保守治疗后并发症的探讨Complication discuss of proximal humeral fractures with non-operative treatment

摘要(Abstract):
[目的]探讨肱骨近端骨折经保守治疗后并发症的发生原因,并提出相应对策。[方法]2010年1月~2013年1月,回顾总结随访资料完整的49例肱骨近端骨折经保守治疗的患者,其中男15例,女34例,平均年龄78岁(67~92岁)。治疗后平均随访21.5个月(10~40个月),采用Constant-Murley评分以及肩关节前屈、外展、外旋、后伸内旋活动度评价肩关节功能。同时采用独立样本t检验来比较并发症组与非并发症组的肩关节功能评分及活动度,并分析并发症原因及所做的相应处理。[结果]49例患者得到随访,9例出现并发症,并发症发生率为18.4%,非并发症组的肩关节功能结果及活动度(除了后伸内旋外)比并发症组有显著的提升。[结论]对老年的肱骨近端骨折患者,通过保守治疗可以获得较好的临床功能预后,但是仍存在一定的并发症发生率。需要充分评估患者的情况,根据骨折类型,医生的经验合理选择治疗方案,尽量减少并发症的发生。
