摘要(Abstract)：

[目的]系统评价一期人工股骨头置换术与闭合复位内固定术治疗老年移位型股骨颈骨折的疗效。[方法]计算机检索MEDLINE(1966～2011年3月)、EMBASE(1966～2011年3月)、Cochrane图书馆(2011第3期)及CBM、CNKI、万方、维普中文数据库(均截止2011年3月),手工检索中、英文已发表或未发表的资料和会议论文并查阅相关文章的参考文献。纳入人工股骨头置换术与闭合复位内固定术治疗老年移位型股骨颈骨折的所有随机对照试验(RCT)。由2名评价员独立提取资料,并对其方法学质量进行评价。对符合纳入标准的研究用RevMan 5.0进行统计分析。[结果]纳入7个随机对照试验,共计1 537例病人。Meta分析显示:与闭合复位内固定术相比,人工股骨头置换术治疗老年移位型股骨颈骨折能降低术后24～36个月的再手术率[RR=0.15,95%CI(0.10～0.24)P<0.000 01]和主要并发症发生率[RR=0.17,95%CI(0.12～0.23)P<0.000 01]。[结论]治疗老年移位型股骨颈骨折,人工股骨头置换术较闭合复位内固定术能降低术后24～36个月的再手术率和主要并发症发生率;两种治疗方法术后12～36个月的病死率差异无统计学意义。但年龄及精神状况是否对手术方式选择产生影响,尚需包含以上两种因素的大样本、多中心随机对照试验的开展。
