肩上方悬吊复合体多处损伤的切开复位内固定Open reduction and internal fixation for multiple injuries of shoulder suspension complex

摘要(Abstract):
[目的]探讨切开复位内固定治疗肩关节上方悬吊复合体(superior shoulder suspensory complex, SSSC) 3、4处损伤的临床疗效。[方法]回顾性分析2016年8月—2021年12月本院开放复位内固定治疗的SSSC 3、4处损伤11例患者的临床资料。评价临床及影像资料。[结果]患者均顺利完成手术,无神经、血管损伤等并发症,随访平均(28.9±3.9)个月。随术后1、3个月和末次随访的时间推移,VAS评分[(7.0±1.5),(3.8±1.5),(1.8±1.0), P<0.001],Constant-Murley评分[(26.6±3.2),(40.9±2.9),(54.4±2.9), P<0.001],肩前屈-上举[(94.3±9.6)°,(127.0±6.4)°,(145.8±6.7)°, P<0.001],外展-上举[(75.7±6.5)°,(92.8±5.3)°,(115.2±5.9)°, P<0.001]和内-外旋ROM[(14.2±2.3)°,(32.4±3.1)°,(59.5±7.1)°, P<0.001]均显著改善,无感染和骨不连等并发症发生。影像方面,术后AHD无显著变化(P>0.05),所有骨折均骨性愈合。[结论]对于SSSC 3、4处损伤,采用切开复位内固定手术治疗能够恢复SSSC的骨性结构和稳定性,肩关节功能恢复良好,建议作为首选治疗方式。
